Youth Soccer Club Liability Waiver, Release of Claims, Assumption of Risk & Indemnification

Agreement

This Liability Waiver, Release of Claims, Assumption of Risk, and Indemnification Agreement

(“Agreement”) is entered into by the undersigned parent or legal guardian (“Parent/Guardian”) on behalf

of the minor participant (“Participant”), in favor of Premiere Training Academy, LLC (“PTA”), on the date

Signed below.

1. Description of Activities

Premiere Training Academy is a youth soccer organization offering training sessions, competitive teams

participation, leagues, tryouts, camps, clinics, tournaments, matches, scrimmages, strength and

conditioning, speed and agility training, classroom or video sessions, travel-related activities, and

related events (collectively, the “Activities”). Participation involves physical exertion, athletic

competition, travel, and inherent risks.

2. Assumption of Risk

The Parent/Guardian and Participant knowingly and voluntarily assume all risks, both known and

unknown, including physical injury, concussion, illness, emotional distress, permanent disability, death,

or property damage arising from participation in the Activities, whether caused by field conditions,

equipment, weather, other participants, coaching instruction, or negligence of PTA or others.

3. Release and Waiver of Liability

To the fullest extent permitted by law, the Parent/Guardian releases and forever discharges PTA, its

owners, officers, coaches, staff, volunteers, affiliates, sponsors, facility owners, and governing bodies

from any and all claims arising out of participation in the Activities, including those caused by

negligence, except where prohibited by law.

4. Indemnification & Hold Harmless

The Parent/Guardian agrees to indemnify and hold harmless PTA from all claims, damages, losses, or

expenses (including attorney’s fees) arising from participation, breach of this Agreement, or actions of

the Participant or Parent/Guardian.

5. Medical Authorization & Responsibility

The Parent/Guardian certifies the Participant is physically able to participate and authorizes emergency

medical treatment if necessary. The Parent/Guardian accepts full financial responsibility for all medical

expenses. PTA does not provide medical insurance.

6. Concussion & Injury Acknowledgment

The Parent/Guardian acknowledges the risk of concussion and agrees to comply with all return-to-play

protocols and medical clearance requirements.

7. Transportation & Travel Disclaimer

The Parent/Guardian acknowledges activities may involve travel and releases PTA from liability related

to transportation unless expressly provided by PTA in writing.

8. Code of Conduct & Club Policies

The Parent/Guardian agrees to comply with all PTA policies. Failure to comply may result in

suspension or removal without refund.

9. Use of Likeness & Media Release

The Parent/Guardian grants PTA the right to use the Participant’s name, image, video, and likeness for

promotional or educational purposes without compensation.

10. Governing Law & Venue

This Agreement shall be governed by the laws of the State of Arkansas.

11. Severability

If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.

12. Entire Agreement

This Agreement constitutes the entire understanding between the parties and supersedes all prior

agreements.

13. Voluntary Execution

The Parent/Guardian acknowledges they have read, understand, and voluntarily agree to this

Agreement.
